SEMAGLUTIDE / U.S. MEDICATION COSTS
Ozempic costswith & without insurance
Ozempic’s self-pay pen price depends on strength. Compare the regular pharmacy price, the temporary new-patient offer and the benefit cap on commercial-insurance savings.
Manufacturer pricing reviewed
Ozempic contains semaglutide and is used for type 2 diabetes and other labeled indications. It is not the Wegovy weight-management product. Savings eligibility requires a prescription for an approved indication. Manufacturer information ↗
Ozempic cost without insurance
Published manufacturer-program prices in USD. These are not universal retail prices or complete care packages. A prescription is required; ask the pharmacy to confirm eligibility and the amount due.
| Pen: 0.25 / 0.5 / 1 mg | $349 Standard self-pay price per program-defined monthly fill: one box containing one pen. Source ↗ |
|---|---|
| Pen: 2 mg | $499 Standard self-pay price per monthly pen fill; do not apply the lower-strength price to this presentation. Source ↗ |
| Introductory pen offer | $199 per eligible fill Two 0.25/0.5 mg fills for eligible new patients through December 31, 2026; later fills use the standard price. Source ↗ |
| Pill: 1.5 / 4 / 9 mg | $149 / $199 / $299 One bottle of 30 tablets at the respective strength. Tablet and pen strengths are not interchangeable. Source ↗ |
Ozempic cost with insurance
Having insurance does not guarantee that a specific prescription is covered. Ask for the patient charge after the plan processes the prescription, then check whether a savings offer can apply.
| Insurance covers your prescription | Request your plan’s exact pharmacy price; coverage, deductibles and supply limits affect the bill. Source ↗ |
|---|---|
| Eligible commercial coverage + savings | As low as $25 per qualifying fill, with maximum savings of $100 / $200 / $300 for a one- / two- / three-month prescription. The three-month option requires an eligible prescription and plan coverage for that quantity. Source ↗ |
| Insurance excludes Ozempic / no insurance | Check the self-pay program above or obtain a retail quote. Cash-program payments cannot be submitted for insurance reimbursement or counted toward deductibles or out-of-pocket limits under the offer terms. Source ↗ |
| Medicare, Medicaid or other government coverage | Government beneficiaries cannot use the commercial copay offer. Check plan-specific benefits and separate self-pay conditions rather than assuming the $25 offer applies. Source ↗ |
Commercial savings-card eligibility generally excludes government beneficiaries and requires other program conditions. A separate manufacturer cash-pay offer can have different rules. Do not assume coupons can be combined or that cash purchases count toward plan spending limits.
WHY “AS LOW AS” IS NOT A QUOTE
An illustrated $75 patient payment
- Hypothetical charge after insurance
- $175
- Maximum one-month savings in this example
- −$100
- Remaining medication payment
- $75
A hypothetical $175 patient charge on an eligible covered one-month fill, reduced by the full $100 savings benefit, leaves $75. This illustrates the cap, not a personal copay estimate. Savings terms ↗
Illustration assumes full eligibility, the entire stated benefit remains available and the pharmacy applies it. It excludes clinical care and other charges.
What changes the total cost?
Match the prescribed pen strength before comparing receipts. Two people buying Ozempic can have different medication prices even before insurance is involved. Also ask whether the quote uses insurance or a self-pay offer: an immediate cash discount does not necessarily reduce what remains to be paid under your insurance deductible. Keep the pharmacy price separate from the cost of the clinician managing your prescription.
Keep care and medication on separate lines
Ask about consultation charges, recurring memberships, follow-up appointments, laboratory work, delivery, injection supplies and taxes. An unlisted charge is not automatically included. Compare the care-provider fee guides, then use the payment budget to add the invoices actually due.
Before you pay
- Is this a pen or tablet quote, at the exact prescribed strength?
- Will this purchase use insurance or the separate cash program?
- Does my plan require prior authorization, and is it already approved?
