TIRZEPATIDE / U.S. MEDICATION COSTS

Zepbound costswith & without insurance

Zepbound prices depend on whether the prescription is for vials, a KwikPen or single-dose pens. Compare self-pay dose prices separately from the commercial-insurance savings card.

Manufacturer pricing reviewed

Zepbound contains tirzepatide. Its presentation-specific prices and approved-use requirements must match the prescription. Mounjaro is a separate branded product, not a substitute chosen by comparing these price tables. Manufacturer information ↗

Zepbound cost without insurance

Published manufacturer-program prices in USD. These are not universal retail prices or complete care packages. A prescription is required; ask the pharmacy to confirm eligibility and the amount due.

Self-pay pricing by product and supply

Vial / KwikPen: 5 mg$399

Vial / KwikPen: 7.5 mg$499 regular; $449 offer

Vial / KwikPen: 10 / 12.5 / 15 mg$699 regular; $449 offer
Per 28-day supply, not the single-dose autoinjector pen price. Source ↗

For the $449 higher-dose offer, the first qualifying purchase receives the offer; subsequent refill purchases must be completed within 45 days of the previous delivery/receipt. Missing that window means the regular price applies. Taxes and supply fees may be additional. Follow the prescribed treatment schedule, not a discount deadline. Source ↗

Zepbound cost with insurance

Having insurance does not guarantee that a specific prescription is covered. Ask for the patient charge after the plan processes the prescription, then check whether a savings offer can apply.

Coverage and savings are separate questions
Covered single-dose pens + eligible commercial cardAs low as $25 for a one-, two- or three-month fill. Savings capped at $100 / $200 / $300 respectively, $1,300 annually and 13 fills. Card expires December 31, 2026. Source ↗
Commercial insurance does not cover single-dose pensEligible patients may pay as low as $499 per 28-day fill through that card program. This is not an uninsured discount; commercial coverage and other eligibility rules are required. Source ↗
No insuranceCompare vial/KwikPen self-pay options through LillyDirect and eligible pharmacy routes. The $299 starting price does not apply to single-dose pens. Source ↗
Medicare, Medicaid or other government coverageCommercial-card benefits exclude government beneficiaries. Confirm any covered prescription or separate program directly with the plan; cash-pay eligibility is a different question. Source ↗

Commercial savings-card eligibility generally excludes government beneficiaries and requires other program conditions. A separate manufacturer cash-pay offer can have different rules. Do not assume coupons can be combined or that cash purchases count toward plan spending limits.

WHY “AS LOW AS” IS NOT A QUOTE

An illustrated $150 patient payment

Hypothetical charge after insurance
$250
Maximum one-month savings in this example
−$100
Remaining medication payment
$150

For an eligible covered single-dose-pen fill, a hypothetical $250 patient charge less the full $100 monthly benefit leaves $150. This example does not apply the commercial card to a vial or cash-pay purchase. Savings terms ↗

Illustration assumes full eligibility, the entire stated benefit remains available and the pharmacy applies it. It excludes clinical care and other charges.

What changes the total cost?

A box of four single-dose pens and one multi-dose KwikPen can both represent four weeks, yet have different payment programs. Ask the pharmacy to identify the presentation on the quote. When comparing a refill offer with insurance, consider the ongoing price if offer conditions are no longer met. Thirteen 28-day fills cover 364 days; twelve cover only 336, so twelve fills should not be labeled a full year of continuous supply.

Keep care and medication on separate lines

Ask about consultation charges, recurring memberships, follow-up appointments, laboratory work, delivery, injection supplies and taxes. An unlisted charge is not automatically included. Compare the care-provider fee guides, then use the payment budget to add the invoices actually due.

Before you pay

  • Does my quote identify single-dose pens, vials or a KwikPen?
  • What regular price applies if a refill offer no longer qualifies?
  • Will supplies, visits, labs or taxes be billed separately?
